Use of Antibiotics Needs to be Reduced, Says Chief Medical Officer of Britain

Britain’s Chief Medical Officer Dame Sally Davis has said that the use of antibiotics should be cut down drastically as it is giving rise to resistant bacteria leading to increased risk of death even from routine surgical procedures. 
Dr Davis went on to add that the rampant use of antibiotics could lead to development of new forms of disease, including an incurable form of gonorrhea. Antibiotics are often given to people with cough, sinusitis and sore throats but Dr Davis said that most forms of the conditions are easily curable without the need for antibiotics.
Dr Davis was backed by a number of experts, including the head of primary care at Health Protection Agency Dr Cliodna McNulty who said that doctors are being pressurized by patients to prescribe them antibiotics even for uncomplicated conditions. 
“Doctors are faced daily with patients who expect antibiotics for uncomplicated infections that will usually get better on their own. This expectation puts a lot of pressure on the doctor to prescribe antibiotics which is often not necessary and cause increased antimicrobial resistance in the long run”, Dr McNulty said. 


 
 

Caterpillar Fungi’s Anti-Inflammatory Properties can be Utilized in Developing Drugs for Number of Conditions

Researchers at University of Nottingham studying the caterpillar fungi, Cordyceps, have found that cordycepin, a drug found in the mushrooms, has anti-inflammatory properties that can be used in developing new cancer drugs as well as in treating asthma, rheumatoid arthritis, stroke damage and renal failure.The research, published today in the academic journal RNA, was led by Dr Cornelia de Moor in the School of Pharmacy. It shows that cordycepin reduces inflammatory gene products in airway smooth muscle cells – the cells that contract during an asthma attack.Several studies have suggested that cordycepin could be an effective drug for a variety of conditions, including cancer, stroke, kidney damage and inflammatory lung disease but until now it was unclear how cordycepin could bring about so many different beneficial effects at the cellular level. 
Dr de Moor said: "We have shown that cordycepin reduces the expression of inflammatory genes in airway smooth muscle cells by acting on the final step in the synthesis of their messenger RNAs (mRNAs) which carry the chemical blueprint for the synthesis of proteins. This process is called polyadenylation. Commonly used anti-inflammatory drugs either work much earlier in the activation of inflammatory genes, such as prednisone, or work on one of the final products of the inflammatory reaction (e.g. ibuprofen).These findings indicate that cordycepin acts by a completely different mechanism than currently used anti-inflammatory drugs, making it a potential drug for patients in which these drugs don't work well. 
"However, it is a surprise that cordycepin does not affect the synthesis of mRNAs from other genes, because nearly all mRNAs require polyadenylation." 
Dr de Moor's research suggests that this is because inflammatory genes can be very rapidly induced and that cordycepin has its many and varied effects by altering the synthesis of other classes of rapidly induced genes as well. If this is true if could be said that cordycepin slows down the rapid cellular responses to tissue damage and may work by preventing the over-activation of these responses which are associated with conditions such as asthma, rheumatoid arthritis, renal failure, cancer and stroke damage. 
However, it also indicates that cordycepin could have adverse effects on normal wound healing and on the natural defences against infectious diseases. 
Dr de Moor said: "We are hoping to further investigate which genes are more dependent on polyadenylation than others and why this is the case, as well as test the effect of cordycepin on animal models of disease. Clinical testing of cordycepin is not in our immediate plans, as we think we first have to understand this drug in more detail before we can risk treating patients with it."

Ancient foot massage technique may ease cancer symptoms


 A study led by a Michigan State University researcher offers the strongest evidence yet that reflexology – a type of specialized foot massage practiced since the age of pharaohs – can help cancer patients manage their symptoms and perform daily tasks.Funded by the National Cancer Institute and published in the latest issue of Oncology Nursing Forum, it is the first large-scale, randomized study of reflexology as a complement to standard cancer treatment, according to lead author Gwen Wyatt, a professor in the College of Nursing.“It’s always been assumed that it’s a nice comfort measure, but to this point we really have not, in a rigorous way, documented the benefits,” Wyatt said. “This is the first step toward moving a complementary therapy from fringe care to mainstream care.”Reflexology is based on the idea that stimulating specific points on the feet can improve the functioning of corresponding organs, glands and other parts of the body.The study involved 385 women undergoing chemotherapy or hormonal therapy for advanced-stage breast cancer that had spread beyond the breast. The women were assigned randomly to three groups: Some received treatment by a certified reflexologist, others got a foot massage meant to act like a placebo, and the rest had only standard medical treatment and no foot manipulation.Wyatt and colleagues surveyed participants about their symptoms at intake and then checked in with them after five weeks and 11 weeks.They found that those in the reflexology group experienced significantly less shortness of breath, a common symptom in breast cancer patients. Perhaps as a result of their improved breathing, they also were better able to perform daily tasks such as climbing a flight of stairs, getting dressed or going grocery shopping.Wyatt said she was surprised to find that reflexology’s effects appeared to be primarily physical, not psychological.“We didn’t get the change we might have expected with the emotional symptoms like anxiety and depression,” she said. “The most significant changes were documented with the physical symptoms.”Also unexpected was the reduced fatigue reported by those who received the “placebo” foot massage, particularly since the reflexology group did not show similarly significant improvement. Wyatt is now researching whether massage similar to reflexology performed by cancer patients’ friends and family, as opposed to certified reflexologists, might be a simple and inexpensive treatment option.Reflexology did not appear to reduce pain or nausea, but Wyatt said that could be because the drugs for combating those symptoms are generally quite effective, so the women may not have reported them to begin with.Although health researchers only recently have begun studying reflexology in a scientifically rigorous way, it’s widely practiced in many parts of the world and dates back thousands of years.“Reflexology comes out of the Chinese tradition and out of Egypt,” Wyatt said. “In fact, it’s shown in hieroglyphics. It’s been around for a very long time.”Wyatt’s co-authors include MSU statistics and probability professor Alla Sikorskii and College of Nursing research assistant Mei You, along with colleagues from Northwestern University and the University of Texas Health Science Center at Houston.

Improving quality of life for the bedridden


Immobile patients are in constant danger of developing pressure ulcers on the skin. Empa, Schoeller Medical and the Swiss Paraplegic Centre have worked together to develop a special sheet that is gentle on the skin and helps to make patients more comfortable 
The skin is the most versatile of our organs: It protects the body from environmental effects, contributes to the body’s immune system and supports metabolic functions such as breathing. The skin is always in action. Lack of movement is anathema to it. If a patient does not move, the ever higher moisture levels, pressure and effects of gravity lead to circulatory disorders. This increases the amount of toxic tissue, resulting in ulcers which can lead to life-threatening complications.
This affects two groups of patients in particular: the elderly and paraplegics. There is an up to 50 per cent higher risk of these persons developing bedsores during a hospital stay, despite all the advances in methods of care. Four out of five paraplegics develop a pressure ulcer at least once in their life.
Medical technology has recognized the problem. There are many approaches to increasing the comfort of those at risk of developing pressure ulcers. But some of these methods are not sufficiently effective, while others – such as mattresses with changing areas of pressure – are still very expensive.
Bed linen is the solution

Anke Scheel, senior physician at the Swiss Paraplegic Centre (SPC) in Nottwil, Canton Lucerne, did therefore not hesitate when Empa asked the center to become involved in a project to develop a new kind of bed linen. "It seemed such a simple solution," recalls Scheel, “that’s what I liked about it.”
The project is led by Siegfried Derler, who works at the Department of Protection and Physiology at Empa in St. Gallen. The trained physicist has devoted several years to research into the skin and friction, and he is currently working on the development of skin-friendly materials and surfaces.
Derler has been researching the medical phenomenon of pressure ulcers since 2006. Back then he launched an initial CTI project for textile pressure ulcer prevention along with the Schoeller Group, a leading international provider of technical textiles. The results were very encouraging, and three years later the partners decided to raise the stakes, inviting the SPC to get involved.
Fewer points of contact thanks to special textile structure

"We evaluated marketable synthetic fibers," explains Derler, "and developed a material with a kind of dot matrix surface." This special textile structure has two advantages: firstly, it produces fewer points of contact and less surface contact with the skin, and secondly, the microscopic spaces between the dots can absorb moisture. What followed was a highly exciting Ping-Pong process between the Empa test labs and the R & D department of Schoeller in Sevelen, Canton St. Gallen. The industrial partner optimized its weaving techniques, and Siegfried Derler’s team tested the new samples on its sophisticated skin models.
By the end of 2009 they were ready: The laboratory results were good enough for the fabric to be tested in hospital beds at the SPC in Nottwil. Twenty paraplegics at the end of their first rehabilitation period offered to act as a test group. Although their skin had already changed as a result of their disability, it had not yet suffered the effects of years of inactivity.
Better circulation and greater comfort

Over a period of eighteen months, experts at Empa and the medical staff in Nottwil regularly checked the patients’ blood circulation as well as the redness, elasticity and moisture levels of the affected parts of the skin. The subjective wellbeing of the subjects was recorded in a questionnaire.
And the results were more than satisfactory: The patients sweated less, their blood circulation improved, and they felt significantly more comfortable than in conventional sheets. "We have shown that our method of bedsore prevention using textiles is effective," says Hans-Jürgen Hübner, CEO of Schoeller Medical. He is currently subjecting the new sheet to tough tests at Schoeller in Sevelen in order to show how it behaves after repeated use and cleaning.
Next spring Schoeller Medical hopes to launch its innovation on a commercial basis. "We are currently building up an international distribution system," says Hans-Jürgen Hübner. He already has one potential customer in the Swiss Paraplegic Centre, which, thanks to the initial positive results, is interested in using the new sheets on a wider basis. "What’s more," says Anke Scheel, "some of our patients would be interested in using the sheets at home.”

Exercise benefits found for pregnancies with high blood pressure


Contrary to popular thought, regular exercise before and during pregnancy could have beneficial effects for women that develop high blood pressure during gestation, human physiology professor Jeff Gilbert said, summarizing a new study by his research team that appears in the December issue of Hypertension, a journal of the American Heart Association.
Gilbert's team observed that placental ischemia-induced hypertension in rats was alleviated by exercise and was accompanied by a restoration of several circulating factors that have recently been shown to be important in causing the high blood pressure associated with preeclampsia.
Hypertensive disorders of pregnancy, such as preeclampsia, are the most common dangerous pregnancy complications, occurring in 5 percent to 8 percent of pregnancies. Globally, preeclampsia and other hypertensive disorders of pregnancy cause 76,000 maternal and 500,000 infant deaths each year, conservatively, according to the Preeclampsia Foundation.
"The data from our study raise the possibility that exercise regimens if started before pregnancy and maintained through most of gestation may be an important way for women to mitigate the risk of preeclampsia," Gilbert said. "There are certainly questions that remain, such as when and how much exercise is required and whether exercise training must begin before pregnancy for these beneficial effects to occur. Moreover, further studies are needed to determine if it can safely be used as a thera­peutic modality for hypertension caused by insufficient blood flow in the placenta.
"But these results are certainly encouraging," he added.
Previous clinical and epidemiological studies have long suggested that regular exercise before and during pregnancy reduced risk of preeclampsia but definitive mechanistic studies on the topic had been lacking until recently. Even though these findings are exciting, there are still safety concerns that need to be evaluated clinically and exercise continues to be discouraged in pregnancies complicated by high blood pressure.
Working in accordance with National Institutes of Health guidelines for animal use, Gilbert's team induced hypertension in pregnant rats by restricting blood flow to the rat placentas and monitored gestation after six weeks of exercise on activity wheels. Animals in test and control groups ran approximately 30 kilometers per week before pregnancy and approximately 4.5 kilometers per week during pregnancy.
Wheel running before and during pregnancy reduced high blood pressure. It also improved circulating concentrations of vascular endothelial growth factor and restored the balance between that factor and soluble fms-like tyrosine kinase 1, which restricts blood vessel growth and function. The study suggests that exercise lowers high blood pressure by maintaining the balance of these two factors that promote proper blood vessel growth and function.
Exercise also improved endothelial cell function and reduced oxidative stress in the hypertensive rat. Fetal weight was not compromised by exercise and there were no obvious signs of fetal stress in rats with hypertension that exercised, Gilbert said.

UGA study finds anxiety linked to chest pain in children


Athens, Ga. - Psychological factors can have as much-or more-impact on pediatric chest pain as physical ones, a University of Georgia study found recently. UGA psychologists discovered pediatric patients diagnosed with noncardiac chest pain have higher levels of anxiety and depression than patients diagnosed with innocent heart murmurs-the noise of normal turbulent blood flow in a structurally normal heart.The UGA research was done in collaboration with Children's Healthcare of Atlanta and Emory University."The fact that these psychological symptoms are higher in noncardiac chest pain patients suggests the psychological symptoms may be playing a role in the presentation of chest pain," said Jennifer Lee, a doctoral candidate in the UGA Franklin College of Arts and Sciences and the study's lead author.The results of the study, which were published Nov. 5 in the Journal of Pediatric Psychology, show a statistically significant increase in anxiety and depression among patients who are later diagnosed with noncardiac chest pain when compared to patients diagnosed with innocent heart murmurs. Lee said it is not clear if the anxiety is a cause of the pain or if pain caused the anxiety in the sample group."The higher levels weren't so high as to cause a clinical diagnosis on their own, but when you contrast the two groups, there were statistically significant differences," said study co-author Ronald Blount, a psychology professor in the Franklin College.The study included 129 patients ages 8 to 18. The group completed surveys prior to diagnosis. All patients were essentially in the same situation-sitting in a cardiology office awaiting their unknown medical diagnosis."We are highlighting that psychology has a part in these symptoms," Blount said, "and screening for psychological, as well as medical factors, is one implication we foresee coming from this investigation."Chest pain can indicate serious medical conditions that require a doctor's evaluation. In adults, pain in the chest is often linked to a cardiac problem. In children, however, less than 2 percent of patients receive a cardiac diagnosis for their pain.The Sibley Heart Center at Children's Healthcare of Atlanta is one of the largest pediatric heart centers in the world, seeing an average of 2,500 new patients with acute chest pain each year."We know up front that 99 percent of these patients will not have a heart complication at all. The trick is, it is not zero, and I cannot miss the one patient with heart disease because the consequences could be catastrophic," said Dr. Robert Campbell, chief of the Sibley Heart Center and division director of the department of pediatrics at the Emory University School of Medicine.The study also observed that noncardiac chest pain patients have a higher level of functional disability-or the inability to participate in everyday activities like running across a football field or making it through a day without a nap. They also spent less time at school and were less involved in extra-curricular activities."These kids also report greater levels of physical symptoms with unclear causes, like joint pain, stomach aches, head aches," Lee said. "These symptoms are believed to be psychological manifestations of stress. Sometimes your brain doesn't tell you that you are stressed out, but your body does, so you will experience symptoms like these."Pediatric patients with noncardiac chest pain also reported higher levels of anxiety sensitivity, a fear of experiencing physical symptoms and additionally fearing that those symptoms are related to a catastrophic health issue."Psychological functioning is heavily related to pain," Blount said. "Pain is a sensory experience, but your attention to one thing or another and your emotions can impact your experience of pain. And how debilitating the pain is for you can be determined by psychological and social factors. That is what we were interested in looking at."Previous studies have shown reducing emotional symptoms can lead to better coping ability with pain, Lee said. "A goal with these patients would be to create a clearer, more comprehensive picture of what is going on and determine a better way to help these kids and adolescents."Campbell said the UGA research helps him serve the majority of his patients who leave his office with a clean bill of health but unexplained chest pain."Most importantly what we've learned is that after a physical evaluation of the patient, we take the time to sit down with the family and find out what they are most worried about so we can address their psychological issues," Campbell said. "We've learned to pay attention and to be confident and reassuring. I need to make it clear to my patients and their parents that I'm not saying you are not having pain, but that the pain is not caused by your heart."

Wandering minds associated with aging cells

UCSF study finds association between attentional state and length of telomeres
Scientific studies have suggested that a wandering mind indicates unhappiness, whereas a mind that is present in the moment indicates well-being. Now, a preliminary UCSF study suggests a possible link between mind wandering and aging, by looking at a biological measure of longevity.
In the study, telomere length, an emerging biomarker for cellular and general bodily aging, was assessed in association with the tendency to be present in the moment versus the tendency to mind wander, in research on 239 healthy, midlife women ranging in age from 50 to 65 years.
Being present in the moment was defined as an inclination to be focused on current tasks, while mind wandering was defined as the inclination to have thoughts about things other than the present or being elsewhere.
According to the findings, published online on Nov. 15 in the new Association for Psychological Science journal Clinical Psychological Science, those who reported more mind wandering had shorter telomeres, while those who reported more presence in the moment, or having a greater focus and engagement with their current activities, had longer telomeres, even after adjusting for current stress.
Telomeres are the DNA-caps that protect the ends of chromosomes, preventing them from deteriorating or fusing with neighboring chromosomes. Telomeres typically shorten with age and in response to psychological and physiological stressors. In research pioneered at UCSF, scientists have discovered that telomere shortness predicts early disease and mortality.
As the study assessed mind wandering and telomeres at the same time, the researchers don't yet know whether mind wandering leads to shorter telomeres, whether the reverse occurs, or some common third factor is contributing to both.
Mindful meditation interventions, which promote attention on the present with a compassionate attitude of acceptance, lead to increases in some aspects of health. Previous studies have found that they are associated with increased activity of an enzyme known as telomerase, which is responsible for protecting and in some cases, replenishing telomeres.
Along with the new UCSF study, these findings support the possibility that a focus on the present may be part of what promotes health measurable at the cellular level, the researchers said.
"Our attentional state—where our thoughts rest at any moment – turns out to be a fascinating window into our well-being. It may be affected by our emotional state as well as shape our emotional state," said Elissa Epel, PhD, associate professor of psychiatry and lead author on the study. "In our healthy sample, people who report being more engaged in their current activities tend to have longer telomeres. We don't yet know how generalizable or important this relationship is."
Moving forward, Epel, along with Eli Puterman, PhD, a psychologist in the UCSF Department of Psychiatry, and colleagues are developing a series of classes to promote more mindful presence, to see if this intervention protects telomere maintenance or even lengthens telomeres.
In the current study, participants self-reported a tendency to mind wander, and were measured for aspects of psychological distress and well-being. The sample was highly educated and had a narrow range of both chronological age and psychological stress (most were low stress), all of which might have contributed to the ability to detect this relationship, Epel said.
